Medical Question

Hi, I have a super sore stomach. Never have a super huge appetite, occasionally I feel hungry but not super often. I sometimes feels nausea but never throw up and my throat feels as though it tightens and my breath sometimes can be hard. And my heart/chest feels as if someone is sitting on me, it just hurts I'm just achey here and there and was wondering if you have an answer for me

Dear,

The symptoms which you are  having are suggestive of GERD (Gastric esophageal reflux disease). It is due to dysfunction of lower esophageal sphincter leading to acid reflux from stomach into esophagus. Because of the reflux of acid you will feel nausea, chest discomfort especially restrosternal burning type of pain. 

As there is more acid you will feel less hungry. During night time due to position of sleeping, acid will go more in to esophagus from stomach, so you may also feel chest tightening and breath holding. Get an Upper GI Endoscopy done to confirm the diagnosis.


Lifestyle modifications you should follow for your symptoms to come down:


  • Losing weight 
  • Avoiding alcohol, chocolate, citrus juice, and tomato-based products,coffee 
  • Avoiding large meals
  • Waiting 3 hours after a meal before lying down
  • Elevating the head of the bed 8 inches

Regarding the treatment aspect of GERD, use prokinetic such as domperidone or levosupride along with proton pump inhibitor rabeprazole or pantaprazole 30 min before food intake.

dear dr. i have malaria from last 4 months.problem is that its not curing (even after 3 times treatment)and 2nd rare thing is that my temp. never gone upto 99f in this 4 months.when last yr when i was typhiod then also my temperature not higher than 98-99f .but it was cured by cefruxime very easily. now i did typhiod s all tests this time its negetive. i have 24 hours fever ,(without temp),mostly in afternoon and evening pain,headache,poor appetite. in the beggening of this problem,doctor found malaria positive (p f).then he gave me lariago 500mg tablets then i feel better after take it.my body pain and fever became less.but then he start malarid 150 mg tablets,my symptoms came back again and after 1 month its became positive again (in card and slide ,both).then i changed the doctor he gave me lumarax80 tabs but not permanently gone.can blood culture solve my problem?if yes then when should i do,now or wait for untill its positive.any other test u want to suggest? thanks
